Rates and insurance

Rates and Insurance

Your care should be shaped by what you need, not by what an insurance plan allows. That is why we work outside of insurance networks and keep every treatment decision between you and your therapist.

Session rates

Individual
$220 to $255
60-minute session
Couples / Family
$255 to $275
60-minute session

Rates vary by clinician based on experience and specialty. Some clinicians utilize longer session times to address more complex treatment goals. Your therapist will confirm your rate before your first session.

Payment

We keep a credit card on file, stored securely and encrypted in our electronic records system, and charge it on the date of service. FSA and HSA cards are accepted and can be used as the card on file.

We ask for 48 hours' notice to cancel or reschedule.

Insurance

We do not participate in any insurance plans. If your plan includes out-of-network benefits, you may be able to receive partial reimbursement for your sessions. On request, we provide a monthly Superbill, an itemized statement with the information your insurance company needs to process an out-of-network claim. We strongly encourage you to advocate for your out-of-network needs with your insurance carrier, especially if you cannot find a provider in your network that offers the level of care we provide at Curo. Ask to speak with a patient advocate or case manager to discuss your coverage options.

We are not covered providers for Medicare or Tricare. Clients with these plans are not eligible for reimbursement.

Questions to ask your insurance company

Before you submit a claim, a short call to your insurance company can tell you what to expect. Ask:

  • Does my policy cover therapy services from an out-of-network provider?
  • What is my deductible for out-of-network services?
  • Once my deductible is met, what percentage of therapy costs is covered?
  • Where do I get the forms to submit an out-of-network claim?
  • How do I submit the claim?
  • How long does reimbursement usually take?

How to submit for out-of-network reimbursement

  1. Check your benefits. Review your policy or call your insurance company to confirm your out-of-network deductible, coinsurance, and reimbursement rate.
  2. Request your Superbill. We provide one monthly or on request. It includes the service codes, diagnosis, dates, fees, and provider Tax ID and NPI that insurers require.
  3. Complete the claim form. Download your insurer's out-of-network claim form from their website or request one from customer service. You will typically need your insurance ID, the provider's name and address, dates of service, services received, and amounts charged.
  4. Submit the claim. Follow your insurer's instructions for submission. Many have an online portal. If mailing, consider certified mail so you can track it.
  5. Follow up. Keep copies of everything you send and the date you sent it. If you have not heard back in 6 to 8 weeks, contact your insurer with your claim number and dates of service.

A note on high-deductible plans: Even if you do not expect reimbursement, submitting claims applies your sessions toward your deductible. That can make later claims eligible for reimbursement within the same year.
